Provides, coordinates, and directs the provision of home nursing care according to physicians orders based on agency policies and procedures, through the competent application of the nursing process.
Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
State Specific-JOB DUTIES/KNOWLEDGE- Arizona State Specific:
Evaluate and assess patients medical condition as ordered and will report any significant change in condition immediately to physician for medical intervention and follow-up.
Records communication in patients medical record, including date, time, and person to which report was given.
-Illinois State Specific:
1.
Assesses and evaluate patients /clients status by:
A.
Establishing and implementing the plan of care B.
Regularly re-evaluating patient/client needs C.
Reviewing and revising the plan of care as necessary at least every 60 days.
2.
Specifies in writing, the assignment for Home health Aides.
Supervises the HHA every 14 days.
Reviews and revises assignment plan as necessary to meet the patients needs at least every 60 days.
3.
Supervises LP(V)Ns at least every 60 days (every 30 days in Indiana) or as required by patients needs.
4.
Complies with accepted professional standards and principles.
Follows all rules/regulations in home care as mandated by state/federal regulations.
-Oregon State Specific:
Perform a LPN supervisory visit every 60 days when the patients condition is stable and predictable, and at least every two weeks when the patients condition is critical and fluctuating.
-Wisconsin State Specific:
HFS 133.
14 Skilled nursing services:
1.
Provision of services.
Skilled nursing services shall be provided by or under the supervision of a registered nurse.
2.
Duties of the Registered Nurse.
The registered nurse shall:
a.
Make the initial evaluation visit to the patient;b.
Regularly reevaluate the patients needs; c.
Initiate the plan of care and necessary revisions; d.
Provide those services requiring substantial specialized care; e.
Initiate appropriate preventive and rehabilitative procedures; f.
Prepare clinical and progress notes; g.
Promptly inform the physician and other personnel participating in the patients care of changes in the patients condition and needs; h.
Arrange for counseling the patient and family in meeting related needs; i.
Participate in in-service programs for agency staff; and j.
Supervise and teach other personnel.
3.
Scope of Duties.
Nurses shall perform only those duties within the scope of their licensure.
4.
Coordination of Services.
A registered nurse shall maintain overall responsibility for coordinating services provided to the patient by the agency.
